A completed W9 form is required before your wholesale account can be approved. Click the “Open W9 Form” button to open the official IRS W9 form. Fill in every field directly on your screen, then save the completed form to your computer, and email it to us at: wholesale@unitedplantgrowers.com
We cannot approve your account until we have received both your registration form above and your completed W9.
